Elmhurst is on the first floor of the 86-35 Queens Boulevard building. The entrance is on the right side of the veranda in front of the building.
Our Elmhurst location is temporarily closed due to damages from Hurricane Ida in early September 2021. We are working on repairing the office. In the meantime, patients may keep their scheduled appointments and can be seen at our nearby Forest Hills location. You may call our Forest Hills office at (718) 268-4720 for any concerns.
